All documents below are required to be submitted to the athletic department prior to participation in official practice.
NWAC Packet
The NWAC Packet is REQUIRED for all incoming athletes and contains important documents and contracts including the NWAC Athlete Disclaimer, Assumption of Risk Contract, Student-Athlete Code of Conduct, and the Release of Records Contract.
- Click HERE to print the form, fill it out in its entirety, turn it into the Assistant Athletic Director's office in OSC 214 prior to the first day of practice.
- Packets can also be found outside OSC 215 to be filled out and turned in.
Physical Exam Form
Must be completed after July 1st and prior to the first practice of the year of participation in intercollegiate athletics at a member college, a student-athlete shall undergo a medical examination and be approved for intercollegiate athletic competition by a medical authority licensed to perform a physical examination by the laws applicable in the state where the exam is conducted. The physical examination shall be valid for twenty-four (24) consecutive months to the date unless otherwise limited by the physician indicating the physical is only good for less than twenty-four (24) consecutive months.
Note: Those licensed to perform physical examinations in the State of Washington include M.D., Doctor of Osteopathy (D.O.), Registered Nurse Practicioner (ARNP), Naturopath (N.D.) and Physician's Assistant (P.A.).
